Scheduling A Visit

You may sign up for our membership on our website by either logging into your account or by creating a registered account. You will select the “Join Membership” option on the left-hand side menu.

You may also call our office and one of our valued team members will get you signed up.

To schedule a member visit, you will log into your account. You will select the “Book a Visit” option on the left-hand side menu. You will then select the Member visit option.

We offer 50% off of the self-pay booking fee ($179) if you have already used your member visits.

Your member visits will roll over for up to 3 months.

Unfortunately, you would not be able to use your membership and insurance.

You can pay with a credit card, Health Savings Account, or Flexible Spending Account. You’ll receive an email receipt for all charges, for your records. If electing an annual membership you may use your CareCredit account to finance your membership.

There are no ETFs on an annual membership. The annual payment is non-refundable. For monthly payment there is a $200 ETF and a true up cost if you choose to cancel prior to the end of the year.
